Job summary

Community Physicians in Illinois is seeking a Patient Care Navigator to act as a bridge between clinicians, facility staff, patients, and families, coordinating seamless transitions of care across settings.

You will provide administrative medical support, manage transition processes, and advocate for patients with emphasis on high-quality, relationship-based care. This is a full-time role with a comprehensive benefits package and travel reimbursement.

Qualifications

• High school diploma or equivalent required.
• Graduate of an accredited Medical Assistant program required.
• Certified Medical Assistant (CMA) preferred.
• 1–3 years of proven experience with EMR/EHR systems and taking vital signs.
• 5–7 years of experience in Skilled Nursing Facilities.
• Strong knowledge of medical terminology.

Responsibilities

• Coordinate transitions of care for patients and families.
• Assist with patient intake, appointments, and documentation.
• Conduct social risk factor assessments to identify unmet health or social needs.
• Attend IDT plan of care meetings and communicate with care teams.
• Assist in data collection for quality metrics and program improvement.
