Job summary

OPCO Skilled Management in Blue Springs, MO seeks a qualified Social Worker for a skilled nursing facility. You will assess psychosocial needs and help access services to meet those needs, following NASW ethics and regulations.

Qualifications

• Certified, licensed, or registered in the state of practice.
• Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field from an accredited school of social work.
• One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.

Responsibilities

• Completes social history and psychosocial assessment identifying social, emotional, and psychological needs; uses MDS to guide care plan.
• Maintains documentation compliant with regulations and company policy including DMS, care plans, progress notes, and discharge planning documents.
• Supports resident rights and self-determination; documents refusals and educates on alternatives.
• Participates in development of interdisciplinary care plan addressing psychosocial needs and goals.
• Provides therapeutic interventions to support transition and adjustment to long-term care.
• Educates residents and families to understand placement and facility issues and refers to social services agencies when needed.
• Coordinates discharge planning and referrals for home care services prior to community return.
• Works with interdisciplinary team to protect resident rights and well-being and address abuse as mandated.
• Collaborates with residents, families, staff to promote proactive self-advocacy for quality of life.
• Assists in planning and directing social service programs; supports survey readiness.
• Engages with nursing and administration to prepare for inspections and implement corrective actions.
