Job summary

Qualifications

• One year of supervised social work experience in a healthcare setting with geriatric residents.
• Bachelor's Degree in Social Work or Human Services field from an accredited school.
• Certified, licensed, or registered in the state of practice required.

Responsibilities

• Completes social history and psychosocial assessment for residents, identifies social, emotional, and psychological needs; uses MDS to guide care plans.
• Maintains accurate documentation per federal/state regulations and facility policies, including advanced directives, MDS, social service notes, and discharge planning.
• Supports residents’ rights and informed consent; educates residents on alternatives and documents refusals appropriately.
• Participates in developing an interdisciplinary care plan addressing psychosocial needs and goals; documents interventions.
• Provides therapeutic support to help residents transition to long-term care and addresses social, emotional, and psychological needs.
• Educates residents and families about placement and services, and refers to appropriate social service agencies when needed.
• Coordinates discharge planning and referrals for home care services prior to community return.
• Collaborates with the interdisciplinary team to promote resident rights and mental well-being; protects against abuse as required by law.
