Job summary

Outfield Healthcare Partners is seeking a licensed social worker to identify psychosocial and emotional needs of residents in a skilled nursing facility. The role involves assessments, care planning and discharge coordination to support residents and families.

The ideal candidate holds a state license and a Bachelor’s degree in Social Work or related field, with a minimum of 1 year of supervised healthcare social work experience, preferably with geriatrics.

Qualifications

• Requires licensed social worker in the practice state.
• Bachelor’s degree in social work or related field from an accredited school.
• Minimum of 1 year supervised social work experience in healthcare, geriatric focus.

Responsibilities

• Complete social history and psychosocial assessment for each resident.
• Maintain documentation compliant with regulations and center policies.
• Support resident rights and inform about therapeutic alternatives where applicable.
• Participate in interdisciplinary plan of care addressing psychosocial needs.
• Coordinate discharge planning and referrals to home care when appropriate.
• Educate residents and families about placement and services; assist with support networks.
