Job summary

Opco-Skilled-Management- is seeking a Social Worker to identify psychosocial, mental, and emotional needs of residents in a skilled nursing facility, aligning with NASW code of ethics and regulatory requirements.

The role emphasizes assessment, documentation, discharge planning, family education, and collaborating with the interdisciplinary team to support resident rights and quality of life. Benefits include Healthcare, Dental, Vision, PTO, and 401K.

Qualifications

• Certified, licensed or registered in the state of practice, required.
• Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field from an accredited school of social work, required.
• One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for each resident identifying social, emotional, and psychological needs.
• Maintains accurate documentation compliant with federal/state regulations and center policies.
• Supports residents’ right to self-determination and documents refusals and alternatives.
• Participates in developing an interdisciplinary care plan addressing psychosocial needs and goals.
• Provides therapeutic interventions to aid adjustment to the facility and resident well-being.
• Coordinates discharge planning and referrals for appropriate home care services.
