Rosaires Care is seeking a Physician to provide comprehensive primary and geriatric care for frail seniors enrolled in a PACE program. The physician will lead clinical care for a panel across clinic, home, and community settings in a patient-centered, team-based model.

Responsibilities include assessing patients, managing chronic conditions, coordinating with specialists, and guiding families in shared decision-making to support dignity, independence, and quality of life.

Qualifikationen

• Medical training and licensure to practice as a physician.
