Job summary

United Community Center in Greendale, WI, is seeking a Registered Nurse to coordinate and manage care for elderly participants enrolled in the Family Care Program. You will perform initial physical assessments, review medical history, assess functional and nutritional status, and develop care plans updated every six months.

You will educate members and families, coordinate medical, nursing, home health services, and equipment, and act as a health consultant to social workers.

Qualifications

• Prepare initial assessment including physical exam, medical history, functional status, and nutrition.
• Ongoing risk management and health assessment per guidelines.
• Develop and revise care plans every six months.
• Provide health education to member and family/caregiver.
• Coordinate services including medical, nursing, home health, equipment and supplies.
• Serve as consultant to social worker regarding health care needs.

Responsibilities
