Responsabilités

• Prepare initial assessments including physical, medical history, functional status, and nutrition
• Ongoing risk management and health assessment per guidelines
• Develop and revise care plans every six months
• Provide health education to member, family, or caretaker
• Coordinate medical, nursing, home health services and equipment
• Consult with social workers on health care needs and concerns
• Manage medications for participants
