Providence PACE in Portland, Oregon seeks a full-time adult-gerontology nurse practitioner or physician assistant to join our interdisciplinary team delivering comprehensive care to older adults across clinics, homes and community settings.

The role emphasizes collaboration with physicians, nurses, social workers, and other professionals to coordinate medical, behavioral health, and supportive services in a patient-centered model.

Qualifications

• Licensed NP or PA with current credentials.
• 1 year of post-graduate clinical experience with geriatric population preferred.
• Training/experience in palliative care or hospice is valuable.

Responsibilities

• Provide primary care in outpatient clinics and community-based settings.
• Collaborate with physicians, nurse practitioners, social workers, pharmacists and other team members.
• Build trust with patients and families while supporting diverse care needs.
