Job summary

Providence PACE is seeking a Nurse Practitioner or Physician Assistant to join our Hillsboro, Oregon team. This full-time, 1.0 FTE role focuses on primary care for older adults within a program that blends clinic and community-based visits.

Ideal candidates have geriatric post-graduate experience; palliative/hospice exposure is valued. Relocation assistance, CME funds, and a comprehensive benefits package are included.

Qualifications

• 1 year of post-graduate clinical experience with a geriatric population preferred.
• Training or experience in palliative care and/or hospice valuable.
• Nurse Practitioner or Physician Assistant credential required.

Responsibilities

• Provide care in outpatient primary care clinics and community-based settings.
• Collaborate closely with physicians, NPs, care managers, social workers, pharmacists, and other team members.
• Build trust with patients, families, and care teams.
