Job summary

Providence PACE (Program of All-Inclusive Care for the Elderly) seeks a full-time Nurse Practitioner or Physician Assistant to join our Portland, Oregon team. This role focuses on comprehensive, whole-person care for older adults in diverse settings, including community-based visits and outpatient clinics.

Ideal candidates have post-graduate experience with geriatrics; training in palliative care or hospice is valuable.

Qualifications

• Geriatric population experience preferred.
• Post-graduate clinical experience in geriatrics or related fields desired.
• Training or experience in palliative care or hospice valuable.

Responsibilities

• Provide care in outpatient clinics and in-home/community settings.
• Collaborate closely with physicians, nurse practitioners, care managers, social workers, pharmacists, and therapists.
• Work within an interdisciplinary team to deliver comprehensive, whole-person care.
• Support patients and families to maintain independence and well-being.
