Job summary

The Staff Pad seeks a Geriatric Social Worker / Case Manager to support older adults in navigating medical, emotional, and social challenges. You’ll build care plans, coordinate resources, and advocate for patients and families in a community-based primary care setting.

Responsibilities include dementia and cognitive support, safe transitions from hospital to home, and collaboration with clinicians to improve outcomes for medically complex seniors.

Qualifications

• Bachelor’s or master’s degree in Social Work or a related field.
• Active Massachusetts LSW, LCSW, or LICSW license in good standing.
• Experience supporting older adults or vulnerable populations.

Responsibilities

• Geriatric psychosocial assessment and care planning with patients and families.
• Coordinate transitions, connect patients with home care and resources.
• Provide behavioral health support and counseling within scope.
• Engage high‑risk patients and participate in care‑improvement initiatives.
• Advocate for patient preferences and facilitate communication among care teams.
