Job summary

Qualifications

• Experience in case management and/or utilization review preferred.

Responsibilities

• Provide case management and care coordination for clients in the facility.
• Coordinate continuum of care activities and utilization of resources.
• Conduct psychosocial assessments and individual/family/group sessions.
• Coordinate treatment and discharge activities as needed.
