Job summary

BoldAge PACE is seeking a Social Work Case Manager to provide comprehensive care coordination and advocacy for program participants and families. You will assess social and environmental needs, develop individualized plans, and connect participants to community resources, working closely with the interdisciplinary team to promote independence and quality of life.

Qualifications

• Bachelor's degree in social work or related field.
• Current License in the required state.
• Experience on an interdisciplinary team preferred.
• 1 year of experience with frail or elderly population preferred; training provided if needed.

Responsibilities

• Coordinate participants' care plans and act as liaison between participants, caregivers, and IDT.
• Assist admissions and discharges to facilities, housing options, and referrals as needed.
• Educate participants and families about PACE services, benefits, and Medicaid/Medicare processes.
• Document clinical services and maintain records per PACE requirements.
• Support participants in maintaining housing and independence through home visits and coordination.
