Qualifications

• Bachelor's or Master's degree in social work from an accredited program.
• Knowledge of the medical case management process and geriatric patient needs.
• Understanding funding resources such as Medicare Part A, B, C, D, Medicaid and other private insurance.
• Imagination and creative ability; ability to work with professionals to coordinate care.
• Medical knowledge and terminology preferred; marketing experience is a plus.

Responsibilities

• Work with individual patients and families toward adjustment to center life and crises.
• Advocate for patients and families in expressing, defining, and resolving grievances.
• Report to the Director of Social Work Services on current needs and status.
• Participate in developing patient care plans with other disciplines.
• Maintain confidentiality of professional information.
