Job summary

Meadows/Rehab at Shannondell is seeking a Licensed Practical Nurse to provide high-quality nursing care under the supervision of the Director of Nursing. The role includes administering medications, coordinating activities of daily living, and contributing to health status assessments and care plans.

The LPN/RN will work with residents across Skilled Nursing, Assisted Living, and Dementia units, ensuring safety, accuracy, and compassionate care in a fast-paced environment.

Qualifications

• PA license as RN or LPN required or eligible for licensure in PA.
• Formal nursing education with ability to perform essential nursing duties.
• Ability to document, assess, and communicate care needs clearly.

Responsibilities

• Provide nursing care to residents in multiple units per physician orders.
• Administer medications and monitor health status changes.
• Develop and monitor care plans and re-assess residents regularly.
• Coordinate with families and other staff to support daily living activities.
• Supervise assigned staff and provide guidance as needed.
• Document care and respond promptly to resident needs and emergencies.
