The Shores of Worthington is seeking an Assistant Director of Nursing (LPN) to join our leadership team. You will oversee the nursing department, ensuring high standards of care and compliance with state and federal regulations, while guiding MDS assessment and care planning.

You will mentor nursing staff in MDS processes, care planning, and clinical best practices, balancing clinical oversight with administrative responsibilities in a collaborative, resident-centered environment.

Formación

• LPN license in Minnesota is required.
• 2+ years nursing leadership in long-term care or skilled nursing.
• Strong knowledge of MDS 3.0, care planning and RAI guidelines.
• Excellent organizational, communication and leadership skills.
• Balance clinical oversight with administrative responsibilities.

Responsabilidades

• Assist the DON in managing the nursing department and ensure regulatory compliance.
• Oversee daily clinical operations, staff supervision, scheduling and support.
• Lead quality improvement initiatives and regulatory surveys.
• Coordinate and oversee the MDS assessment process for accuracy and timeliness.
• Collaborate with interdisciplinary team to develop individualized resident care plans.
