Job summary

WelbeHealth in New Jersey is seeking a Home Care Coordinator to support in-home assessments and coordinate personal care for participants enrolled in our PACE program. Reporting to the Home Care Manager, you will arrange, assess, and oversee home care services in participants’ homes, blending remote work with on-site IDT meetings and assessments.

The role requires an active NJ RN license and at least three years of case management or nursing in care of frail or elderly populations.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in New Jersey required.
• Minimum of three years of case management or nursing experience in a clinical or home setting with a frail or elderly population.
• Nursing knowledge to care for frail, elderly participants and manage complex clinical conditions preferred.

Responsibilities

• Coordinate incoming calls related to participants, Physicians, and agency services regarding Physician orders, participant questions, and referrals
• Communicate with participants by phone and coordinate effectively with nursing, therapy, aides, social services, and physicians regarding schedule changes, test results, and other related updates
• Collaborate with Home Care Services staff to track and monitor home care delivery and scheduling hours
• Partner with the Marketing team to support the enrollment of prospective participants into the program
• Assist with staffing and scheduling activities, soliciting, and manager input
• Participate in end-of-life care, coordination, and support
