Job summary

Molina Healthcare is seeking a qualified Home-Based Long-Term Care Screening Coordinator in Milwaukee, WI. This hybrid role requires monthly in-person meetings at the Molina office and travel to member locations as assigned.

You will support LTCFS processes for older adults and individuals with disabilities, collaborating with the ICT and family to complete screenings, and ensure data accuracy and timely documentation per guidelines.

Qualifications

• At least 1 year experience in health care supporting adults with disabilities or the frail elderly.
• LTCFS certification is required.
• Bachelor’s degree or applicable nursing/license requirements may apply by state.
• Valid driver's license with reliable transportation for travel.
• Ability to support staff development and balance team dynamics.
• Experience with data management, project coordination, and training.
• Ability to explain and teach complex material in groups and individually.
• Excellent verbal and written communication skills.
• Proficiency in Microsoft Office.

Responsibilities

• Schedules appointments with members to complete LTCFS assessments.
• Uses Wisconsin LTCFS to gather information on member status and supports.
• Completes LTCFS collaboratively with member, family, and ICT.
• Coordinates with ICT to ensure enrollment and condition screens are completed.
• Ensures data consistency between assessments and member records.
• Uses spreadsheets to track LTCFS timeframes.
• Submits timely and complete documentation per guidelines.
• Attends meetings and training to stay current on processes and tools.
• Participates in quality assurance activities and audits.
• Maintains LTCFS certification by CST testing.
• Applies professional judgment while respecting member privacy.
