Job summary

Helping Hands Reentry Outreach Centers in Portland, Oregon seeks a Case Manager for the Bybee Lakes HOPE Center. The role builds supportive relationships, helps participants access resources, and supports goal setting toward stable housing.

The position is full-time, pays $22.71 per hour, and offers medical, dental, vision, PTO, holidays, and a 401(k). Some early mornings, evenings, or weekends may be required.

Qualifications

• High school diploma or equivalent is required.
• At least one year of case management or social services experience is preferred.
• Strong interpersonal, verbal, and written communication skills are essential.

Responsibilities

• Build rapport with participants and understand their needs through conversations and records review.
• Meet with assigned participants monthly to develop and update IRPs, set goals, and review progress.
• Help participants navigate program requirements and access housing, healthcare, and other services.
• Coordinate with professionals and community partners involved in participants’ success.
• Respond to crises using trauma‑informed de‑escalation approaches and connect participants with timely resources.
• Maintain an active network of community resources and protect participant confidentiality.
