2nd Chance Clinic in Somerset, KY seeks a full-time Targeted Case Manager and Medical Staff Associate to support patients through assessments, care planning, and service integration.

Qualifications

• Bachelor's degree in human services plus 2 years' experience in behavioral health case management.
• Background check clearance required.
• BLS, non-violent crisis intervention, and suicide intervention certification a plus.
• Strong oral and written communication; proficient with word processing, email and Internet usage.
• Maintain continuing education units per state regulations for targeted case managers.

Responsibilities

• Complete new patient assessments and biopsychosocial screenings.
• Determine level of care and discuss with patient to ensure understanding.
• Plan services based on assessments and facilitate access to needed services.
• Provide psychoeducation and information to patients and act as a positive role model.
• Maintain confidentiality and comply with regulatory standards.
• Coordinate with outside agencies for continuity of care.
• Document thoroughly and ensure timely record filing and authorization.
• Assist physicians with administrative duties and clinic operations.
• Organize, schedule and manage appointments and referrals.
• Handle billing forms, insurance prior authorizations, and lab arrangements.
