Job summary

2nd Chance Clinic is seeking a full-time Targeted Case Manager and Medical Staff Associate for our Somerset, KY office. You will conduct patient assessments, determine care level, and coordinate services and follow-up care.

The role requires a Bachelor's in human services and at least 2 years of behavioral health case management, with strong communication and organizational skills. Certifications in BLS, crisis intervention, and suicide prevention are a plus.

Qualifications

• Bachelor’s degree in human services and 2+ years in behavioral health case management required.
• Provide information for a complete background check.
• BLS, non-violent crisis intervention, and suicide intervention certifications are a plus.
• Strong verbal and written communication, basic computer skills, and professionalism.
• Maintain continuing education units per state regulations for targeted case managers.
• Complete Targeted Case Management Core and SUD trainings within 6 months of employment.
• Clinical, clerical, or medical experience preferred.

Responsibilities

• Complete new patient assessments and biopsychosocial screenings.
• Determine level of care and discuss with patient.
• Explain patient rights regarding confidentiality.
• Plan services and ensure follow-up to patient.
• Provide psychoeducation and resource information.
• Foster relationships with internal and external agencies.
• Maintain complete, accurate documentation per standards.
• Maintain client confidentiality per regulations.
• Respond to patient grievances per policies.
• Attend quarterly staff meetings and monthly departmental meetings.
• Office organization and administration.
• Perform observed urine collection (if trained).
• Venipuncture (if trained).
• Interview patients and document medical history.
• Organize and schedule appointments; update records and authorizations.
• Arrange laboratory services; coordinate billing forms.
• Report issues to supervisors and assist physicians with administrative duties.
• Other duties as assigned.
