This full-time, in-office role requires a Bachelor’s in social services (or HS diploma with two years’ experience), tech‑savvy skills, a valid driver’s license, and ability to lift up to 50 lbs. Starting wage is $23/hour with potential raises based on tenure.

Qualifications

• Bachelor's Degree in social services or related field with at least one year of experience in supportive housing coordination.
• High School Diploma or GED with two years of experience in supportive housing coordination.
• Self-starter with good time management and proactive problem solving.
• Flexible schedule and tech-savvy (MS Office, Zoom, PC, Android).
• Valid driver’s license and vehicle required.
• CPR/First Aid ability and ability to lift up to 50 lbs.

Responsibilities

• Conduct assessments and interviews to determine eligibility.
• Develop and update action plans, case notes, and related documents.
• Provide life skills development to improve independence and quality of life.
• Document care, follow support plans, and collaborate with internal/external teams.
• Refer clients to medical, mental health, and other services.
• Coordinate SSI/SSDI, Medicaid, Medicare, SNAP applications and re-determination.
