180 Degrees, Inc. seeks a Re-entry Case Manager for Clifton Place in Minneapolis, MN.

This full-time, on-site role supports adult men transitioning from incarceration, guiding them to housing, income, and ongoing support with trauma-informed, person-centered practice. You will provide intensive case management, help residents comply with court conditions, connect to housing, healthcare, and employment resources, and build exit plans.

Qualifications

• Bachelor’s degree in human services or related field required.
• Two years of case management experience in criminal justice, re-entry, or homelessness.
• Valid driver's license and insurance; ability to travel to partners.
• Ability to pass DHS background screening.
• Strong written and verbal communication and documentation skills.

Responsibilities

• Provide intensive, ongoing case management to residents at Clifton Place.
• Help residents stay compliant with court-ordered conditions and legal requirements.
• Assist residents in accessing housing, employment, healthcare, and related resources.
• Build realistic exit plans covering housing, income, and ongoing support.
• Apply trauma-informed, person-centered, and accountability-based practices.
• Maintain safety, structure, and program expectations while supporting long-term stability.
• Establish consistent, trusting relationships with residents navigating reentry.
