Job summary

Innovate-Rehab-and-Wellnes is seeking a Licensed Physical Therapy Assistant (PTA) to join our Skilled Nursing Facility team. You will work under the direction of a licensed Physical Therapist to help patients improve mobility, regain strength, and increase function.

This full-time role offers a Monday – Friday schedule with flexible hours, a competitive hourly rate based on experience, and a supportive, collaborative work environment focused on patient-centered care and clinical outcomes.

Qualifications

• Associate degree from an accredited PTA program and a valid state PTA license.
• Rehab/clinical experience preferred; new grads welcome.
• Strong communication and interpersonal skills; CPR certification preferred.

Responsibilities

• Follow treatment plans created by the PT and lead patient exercises.
• Monitor patient progress and document records; report changes to the PT.
• Collaborate with the care team to ensure coordinated treatment.
• Uphold ethical, professional standards in all interactions.
