Job summary

Innovate Rehab and Wellness is seeking a Licensed Physical Therapy Assistant (PTA) for a full-time position in a skilled nursing facility. You will work under the direction of a licensed Physical Therapist to help patients improve mobility, regain strength, and increase function, delivering individualized treatment plans with hands-on, patient-centered care.

Qualifications include an associate degree from an accredited PTA program and a valid state PTA license; CPR certification preferred.

Qualifications

• Valid state PTA license.
• Rehab/clinical experience preferred (new grads welcome).
• Strong communication and interpersonal skills.
• CPR certification preferred.

Responsibilities

• Implement patient-specific treatment plans created by the PT.
• Lead patients through exercises, mobility training, and rehab activities.
• Educate patients on safe techniques and assistive devices.
• Monitor and document patient progress; report changes to the PT.
