Job summary

Innovate Rehab and Wellness seeks a Licensed Physical Therapy Assistant (PTA) to work full-time in a skilled nursing facility. You will assist the Physical Therapist in implementing patient-centered treatment plans, guide patients through exercises, and educate them on safe techniques and devices.

Collaboration with the care team ensures coordinated care and strong clinical outcomes. The role emphasizes one-on-one therapy focus, flexible scheduling, and a supportive team culture.

Qualifications

• Associate degree from an accredited PTA program.
• Valid state PTA license.
• Rehab/clinical experience preferred (new grads welcome).
• Strong communication and interpersonal skills.
• Commitment to high-quality, patient-centered care.
• CPR certification preferred.

Responsibilities

• Implement patient-specific treatment plans created by the PT.
• Lead patients through exercises, mobility training, and rehab activities.
• Educate patients on safe techniques and assistive devices.
• Monitor and document patient progress; report changes to the PT.
• Maintain accurate, compliant therapy records.
• Collaborate with the care team for coordinated treatment.
