Job summary

Eventide Home Healthcare in York/Fillmore/Saline Counties, NE is seeking a Physical Therapist Assistant to work full time on-site from 8:00 a.m. to 4:30 p.m. The PTA provides physical therapy services under a licensed Physical Therapist, serving seniors in home health, nursing homes, and contracted facilities.

The ideal candidate holds a Nebraska PTA license, CPR, and a valid driver’s license with a clean record. Prior geriatric PTA experience is preferred.

Qualifications

• AGE REQUIREMENT: At least age 18.
• EDUCATION: Graduate of an accredited Physical Therapist Assistant school.
• EXPERIENCE: One year experience as PTA, preferred. Geriatric experience, preferred.
• CREDENTIALS: PTA license in good standing, CPR, Nebraska driver’s license and a good driving record. All licenses, certifications, and permits must be kept up to date. Must follow Eventide vaccination requirements.

Responsibilities

• Reviews senior’s/client’s chart and obtains current information; seeks guidance from supervising Physical Therapist.
• Recognizes abnormal physiologic changes and reports them to the supervising Physical Therapist.
• Performs Physical Therapy treatment procedures as delegated by supervising Physical Therapist per plan of care.
• Develops and implements the senior’s/client’s plan of care in collaboration with the physician and other health care professionals according to agency procedures and state/federal regulations.
• Participates in senior/client care conferences and provides input to the plan of care.
• Maintains a schedule that allows coverage in all Rehab programs as needed.
• Responds promptly to frequent and overlapping needs of seniors/clients/caregivers/team members/managers.
• Maintains written documentation reflecting department standards.
• Communicates effectively with families/caregivers and the health care team to meet goals.
