Job summary

Eventide Home Health in Lincoln, NE is seeking a Physical Therapist to work full-time, 8:00 a.m. to 4:30 p.m., providing PT services to seniors in home health.

The PT will evaluate clients, develop and implement plans of care, and coordinate with physicians and care teams to maximize functional independence. The ideal candidate holds a Nebraska Physical Therapist license in good standing and CPR certification, with a valid driver’s license and auto insurance.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Licensed Physical Therapist in good standing in Nebraska required.
• CPR certification required.
• Valid Nebraska driver's license and auto insurance required; driving record acceptable.

Responsibilities

• Evaluates clients using appropriate diagnostic tests for muscles, nerves, joints and function.
• Performs PT treatment procedures or delegates to a licensed PTA as appropriate after evaluation.
• Develops and implements the client's plan of care with physicians and team, following procedures and regulations.
• Documents care provided, progress toward goals, and changes in condition per policy and state/federal rules.
• Communicates changes to physician and nursing staff; coordinates care with the team.
• Designs and evaluates therapeutic exercise programs and trains family/caregivers in the program.
