Qualifications

• Bachelor's degree in Physical Therapy required.
• One year experience as a Physical Therapist preferred; geriatric experience preferred.
• Nebraska Physical Therapist license required; CPR required; valid Nebraska driver's license with a good driving record; auto insurance required; vaccination requirements per Eventide.

Responsibilities

• Evaluates clients using diagnostic muscle, nerve, joint and functional ability tests.
• Performs PT treatment procedures or delegates to a licensed PTA based on evaluations.
• Develops and implements the client's plan of care with physician and health care professionals.
• Documents care provided, progress toward goals, and changes in condition per policies and regulations.
• Reports significant changes to physician and nursing staff.
• Coordinates care with other team members involved in the care.
• Designs and evaluates therapeutic exercise programs and instructs family/caregiver/Home Health Aide in carrying out the program.
• Obtains orders for PT plan of care and any changes as needed.
• Ensures client records are complete and reflect coordination of care.
