CHRISTUS HomeCare in Tyler, Texas is seeking a Full-Time Physical Therapist to join our compassionate team and provide in-home therapy, covering the Canton area. You will assess functional status, mobility, endurance, and pain to determine and implement an effective treatment plan, and monitor progress.

Licensed PT in Texas is required; a valid driver’s license and vehicle insurance or access to transportation are also needed. CPR certification (or completion within 90 days) is preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsabilités

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice and in coordination with the health care team.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs patient and family about plan of care, limitations, and progress.
• Visits patients according to Plan of Treatment and documents progress notes.
• Completes patient evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
