Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in developing the plan of care.
• Provides services within the scope of practice as defined by state laws; coordinates with health care team.
• Observes and reports patient responses to treatment and changes to supervising staff.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family about plan of care, limitations, and progress toward goals.
• Visits patients according to the plan of treatment and submits progress notes per policy.
• Completes patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation and frequency of care.
