CHRISTUS HomeCare in Tyler, Texas, is seeking a Full-Time Physical Therapist to join a dedicated team delivering quality home health care in the Hawkins area. The role emphasizes assessments, plan of care development, and interventions aimed at improving mobility and function.

The ideal candidate will hold current state licensure, possess a valid driver's license with reliable transportation, and have CPR certification (or be able to obtain within 90 days).

Qualifications

• Current Physical Therapy licensure in the state of practice.
• Valid driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates patients' functional status and physical therapy needs and assists in developing the therapy plan of care.
• Provides services within the scope of practice and coordinates with the health care team.
• Monitors patient response to treatment and reports changes to the supervising nurse/physician.
• Treats patients to relieve pain and restore function while maintaining professional standards.
• Informs patients and families about plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and documents progress per policy.
• Completes evaluations and develops PT plan of care within state guidelines.
• Reports outcomes, goals, and projected frequency of care.
