Job summary

Centerville Post Acute in Centerville, OH is seeking a licensed Physical Therapist Assistant (PTA) to join our rehabilitation team. You will work under a licensed Physical Therapist to help residents restore mobility, build strength, reduce pain, and achieve independence.

Ideal candidates have an accredited PTA program degree, Ohio PTA license, CPR certification, and at least 2 years of PTA experience in skilled nursing or post-acute care.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Ohio PTA license and CPR certification required.
• Minimum 2 years PTA experience in skilled nursing/post-acute care preferred.

Responsibilities

• Administer PTA treatments and therapeutic exercises per plan of care.
• Assist residents in mobility, strength, balance, endurance, and independence.
• Monitor progress and communicate changes to supervising PT.
• Document treatments and progress in the EMR.
• Educate residents and families on exercises, safety, and device use.
• Assist with transfers, gait training, and functional mobility activities.
• Participate in interdisciplinary care planning meetings.
• Maintain clean, safe treatment areas and equipment.
• Follow facility policies and Medicare documentation requirements.
• Collaborate with nursing, OT, ST, physicians and the care team.
