Job summary

Centerville Post Acute is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team. Under the supervision of a licensed Physical Therapist, the PTA helps residents restore mobility, improve strength, reduce pain, and achieve independence.

The ideal candidate is licensed in Ohio with CPR certification and at least 2 years of PTA experience, preferably in skilled nursing or post-acute care. Collaborative, patient-centered care is our priority.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Physical Therapist Assistant license in the State of Ohio.
• CPR certification required.
• Minimum of 2 years of Physical Therapist Assistant experience preferred, with skilled nursing or post-acute care experience strongly preferred.

Responsibilities

• Administer physical therapy treatments and therapeutic exercises as directed by the supervising Physical Therapist.
• Assist residents in improving mobility, strength, balance, coordination, endurance, and functional independence.
• Monitor resident progress and communicate changes to the supervising Physical Therapist.
• Accurately document treatments, resident responses, and progress in the electronic medical record.
• Educate residents and family members on exercise programs, safety precautions, and proper use of assistive devices.
• Assist with transfers, gait training, therapeutic activities, and functional mobility exercises.
• Participate in interdisciplinary care planning meetings.
• Maintain treatment areas, equipment, and supplies in a clean and safe condition.
• Follow all facility policies, state and federal regulations, and Medicare documentation requirements.
• Collaborate with nursing staff, occupational therapists, speech therapists, physicians, and other members of the healthcare team.
