Job summary

Centerville Post Acute in Centerville, OH is seeking a licensed Physical Therapist (PT) to join our rehabilitation team in a full-time post-acute care setting. You will evaluate residents, create individualized treatment plans, and provide skilled PT services focused on maximizing independence and quality of life.

The role requires a physical therapy license in Ohio, CPR certification, and at least 2 years of PT experience preferred in skilled nursing or post-acute care.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.

• Establish measurable goals and adjust care plans based on progress.
• Educate residents, families, and caregivers on exercises, safety, and discharge recommendations.
• Complete timely clinical documentation and participate in care planning meetings.
• Collaborate with physicians, nurses, and other therapists to optimize care.
