Job summary

Centerville Post Acute in Centerville, OH is seeking a dedicated Physical Therapist to join our rehabilitation team. You will evaluate residents, design individualized treatment plans, and deliver skilled PT services to maximize independence and quality of life.

Collaboration with physicians and the interdisciplinary team is essential. The ideal candidate is licensed in Ohio, with CPR certification and a minimum of 2 years of PT experience, preferably in post-acute or skilled nursing settings.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Ohio Physical Therapist license.
• CPR certification required.
• Minimum of 2 years physical therapy experience preferred, with skilled nursing or post-acute care experience strongly preferred.
• Experience with electronic medical records (EMR) preferred.

Responsibilities

• Evaluate residents and develop individualized PT treatment plans.
• Provide therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence.
• Establish measurable treatment goals and modify care plans based on resident progress.
• Educate residents, families, and caregivers on exercises, safety techniques, and discharge recommendations.
• Complete timely and accurate clinical documentation per facility, state, federal, and payer requirements.
• Participate in interdisciplinary care planning meetings.
• Collaborate with physicians, nurses, and other therapy professionals.
• Recommend adaptive equipment and environmental modifications as appropriate.
• Ensure compliance with Medicare, Medicaid, and rehabilitation documentation guidelines.
