Job summary

Centerville Post Acute in Centerville, OH is seeking a licensed Physical Therapist to join our rehabilitation team. The PT will evaluate residents, create individualized plans, and deliver skilled therapy to improve mobility and independence.

Requirements include Ohio PT license and CPR; two years of post-acute experience is preferred. Excellent communication and teamwork are essential in our resident-centered care model.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the State of Ohio.
• CPR certification required.

Responsibilities

• Evaluate residents and develop individualized PT treatment plans.
• Provide skilled interventions to improve mobility, strength, balance and independence.
• Collaborate with physicians and the interdisciplinary team.
• Educate residents and families on exercises and safety techniques.
• Document clinical activities per regulatory guidelines.
• Participate in care planning meetings and quality initiatives.
