Job summary

CommuniCare Rehab at Greenfield Healthcare Center seeks a Physical Therapist to evaluate and treat residents and to develop individualized treatment plans under physician orders.

You will implement plans through direct therapy, educate on care plans to the team, supervise PTAs, and participate in interdisciplinary meetings, staff training, and family education as appropriate.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.

Responsibilities

• Evaluate, review and screen patients' medical charts and evaluations under physician order and develop a treatment plan.
• Implement the plan through direct treatment and education of the plan of care to all team members, including supervision of PTAs.
• Attend/lead interdisciplinary meetings to relay clinical findings and make recommendations to rehab team; train staff and families when appropriate.
