Job summary

CommuniCare Rehab in Sellersburg, IN is seeking a Physical Therapist (PT) for a full-time role at Sellersburg Healthcare Center. You will evaluate patient charts, develop a tailored treatment plan, and deliver direct therapy to residents.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
