Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, invites applications for a Full Time Physical Therapist (PT) at Greenfield Healthcare Center in Greenfield, IN. You will evaluate residents, develop and implement treatment plans, and provide supervision to PTAs within an in-house rehab team.

This role offers the chance to work across centers, delivering compassionate care and contributing to interdisciplinary meetings with staff and families.

Qualifications

• A Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, review, and screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
