Job summary

CommuniCare Rehab in Greenfield, IN, is seeking a PRN Physical Therapist (PT) for the Greenfield Healthcare Center. You will evaluate patients, develop individualized treatment plans, and provide direct care while coordinating with an in-house rehab team.

• Evaluate, review, and screen patient charts and develop a treatment plan per physician orders.
• Implement treatment plan through direct care and supervise PTAs as needed.
• Attend and lead interdisciplinary meetings and educate the rehab team and families when appropriate.
