CommuniCare Rehab is seeking a PRN Physical Therapist to provide in-house therapy at Greenfield Healthcare Center. The role involves evaluating residents, developing individualized treatment plans, and delivering direct care while supervising PTAs.

Engage in interdisciplinary meetings and family training to ensure continuity of care. The position offers flexible scheduling, opportunities to work across multiple centers, and a supportive environment with a focus on resident outcomes and staff

Qualificações

• Requires a Physical Therapy degree from an accredited program.
• Must hold a valid, or pending, State Physical Therapy License.
• Clinical decision making and plan development for residents.
