Tice Valley Post Acute is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team in a full-time capacity. This on-site role offers a pay rate of $55.00–$60.00 per hour, DOE, and involvement in a dynamic interdisciplinary team dedicated to resident-centered care.

Join us in Walnut Creek, CA to evaluate residents, develop individualized plans, and support independence through therapeutic activities, adaptive devices, and education for residents and caregivers.

Qualifications

• Strong clinical assessment and treatment planning required.
• Excellent documentation and communication skills.
• Ability to collaborate with physicians, nurses, CNAs, and therapists.

Responsabilités

• Evaluate residents and develop individualized OT plans.
• Provide OT services to improve strength, mobility and ADLs.
• Implement interventions based on care plans and resident needs.
• Assist with activities of daily living and safety optimization.
• Monitor progress and adjust plans as needed.
• Document evaluations, treatments and outcomes accurately.
