Qualifications

• Current Physical Therapist license - California.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential per physician orders and facility policies.
• Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and function.
• Provide skilled PT services and monitor progress toward goals.
• Educate residents, families, and staff; train Restorative Aides as needed.
• Supervise PTAs in resident care per state practice act.
• Communicate progress and plans with physicians, nursing staff, and families.
• Document timely, accurate, and compliant with regulations and payor requirements.
