Job summary

Volare Health LLC is seeking a Licensed Practical Nurse (LPN) for South Hills Rehabilitation Center in Eugene, OR. This full-time evening shift role offers competitive pay based on licensed experience and a supportive nursing team in a skilled nursing and rehabilitation setting.

Newly licensed LPNs are welcome to apply. Responsibilities include direct patient care, care planning, documentation, and supervision of CNAs within a compliant, compassionate environment.

What You'll Do

As an LPN, you will play an important role in providing safe, compassionate, and individualized care to our residents. Responsibilities include:

• Assessing resident needs and monitoring changes in condition


• Developing and implementing individualized care plans


• Providing direct nursing care and ensuring residents' needs are met


• Supervising CNAs and other care team members in the delivery of nursing services


• Maintaining accurate and timely clinical documentation


• Following applicable state and federal regulations, facility policies, and nursing standards


• Communicating effectively with residents, families, providers, and the interdisciplinary care team


• Advocating for residents and helping promote their comfort, dignity, and quality of life

What We're Looking For

• Current, active Licensed Practical Nurse (LPN) license in the state of Oregon


• Ability to assess resident needs and respond appropriately to changes in condition


• Knowledge of individualized care planning and nursing practices


• Ability to supervise and work collaboratively with CNAs and other healthcare team members


• Strong communication and organizational skills


• Compassion, dependability, and a genuine desire to make a difference in the lives of others


• Commitment to following state and federal regulations governing skilled nursing care
