Qualifications

• Bachelor's degree in Physical Therapy; Masters/Doctorate preferred.
• Licensed to practice Physical Therapy in the state; maintain licensure.
• Proficient computer skills; experience with Casamba and Point Click Care.
• Ability to read technical procedures and policy manuals.
• Strong communication with patients, families, and staff.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop and obtain approval for a treatment plan from the referring physician.
• Treat patients per the physician treatment plan.
• Supervise PT assistants and ensure compliant patient care.
• Document evaluations and daily progress notes per policy.
• Instruct families and nursing staff in maintenance and discharge planning.
• Participate in discharge planning and care conferences as needed.
• Report equipment issues and maintain department safety.
