Job summary

Qualifications

• Active, unrestricted medical license in coverage area.
• MD or DO required; multi‑state licenses preferred.
• Experience delivering in-home health evaluations.

Responsibilities

• Perform in-home health evaluations, including medication and medical history review.
• Engage with members and build trusted relationships to improve health outcomes.
