Job summary

Arkansas Hospice, Inc. is seeking a Registered Nurse Case Manager (RN) to oversee and participate in clinical interventions for an assigned hospice patient caseload.

The role emphasizes collaboration with an interdisciplinary team to meet patient and family needs. The RN will manage care after admission, assist in admissions, ensure policy compliance, and maintain ongoing updates to the care plan while coordinating with physicians and community resources.

Qualifications

• Graduation from an accredited school of nursing is required.
• AR RN license is required; BSN preferred.
• Valid Arkansas driver’s license and auto insurance required.

Responsibilities

• Manages patient care after the initial admission and assignment to a designated RN Case Manager.
• Assists in the admission process as requested.
• Interprets Arkansas Hospice, Inc. policies for nursing staff to assure understanding and compliance.
• Coordinates with interdisciplinary team members for the plan of care and ongoing management.
• Updates the Plan of Care to reflect patient status and changing needs for care.
• Facilitates effective written and verbal communication with patients, families, and other providers.
• Maintains communication with attending physician and community resources for patient care.
• On-call rotates to provide 24-hour service to patients and families.
