Arkansas Hospice, Inc. is seeking a Registered Nurse Case Manager (RN) in North Little Rock, AR. The role oversees and participates in clinical interventions for an assigned caseload of hospice patients, coordinating care with an interdisciplinary team.

Requires AR RN licensure, BSN preferred, and at least one year of nursing experience in a relevant setting. The position offers full-time benefits and a supportive, patient-centered work environment.

Qualifications

• Licensed as a Registered Nurse (RN) in Arkansas.
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of one year nursing experience in a hospital, home health agency, hospice, or related program.

Responsibilities

• Manage patient care after initial admission and assignment to an RN Case Manager.
• Assist in the admission process as requested.
• Interpret Arkansas Hospice, Inc. policies for staff and others to ensure understanding and compliance.
• Coordinate with interdisciplinary team to manage the plan of care for patients/families.
• Update the Plan of Care regularly and keep team informed of patient status and needs.
• Communicate effectively with patients, families, and healthcare providers.
